Where to Get Forms

- Post Offices
- Public Libraries
- IRS – Call 1-800-829-3676
- Internet: Click here, then type in the number of the form on the second box on the left.
Common Forms
Form W-4 – you give your employer this form to show how much money to withhold from your paycheck for your taxes
Form W-2 -- your employer gives you this form at the end of the year to use with your taxes
Form 1040 or Form 1040 EZ -- the forms to file federal income taxes
Form 1040X -- you can use this form to change a mistake on a tax return you already sent in. You have up to three years to make changes.
